In order to obtain a passport, it is mandatory to appear at the Embassy of Georgia in Washington DC in person by scheduling an appointment in advance

The following documents must be presented in order to receive a biometric passport:
In order for a passport to be issued to minors under the age of 16, a letter of consent is necessary from both parents. If the minor is 16 years old, a letter of consent is necessary from only one parent. If a parent(s) appears at the Consulate General in person, a letter of consent will be completed in the presence of a consular officer. In the case that a parent or legal guardian is not able to visit the Consulate General, the letter of consent may be certified by a notary office;
1) Copies of parent’s passports;
2) A minor born in Georgia must present a copy of a Georgian Birth Certificate. However, if the applicant is born in a foreign country, an apostilled copy of Birth Certificate must be presented (list of Issuing authorities of apostilles);
3) A money order in the amount of 25 USD written out to the Embassy of Georgia in Washington DC;
4) A prepaid envelope with a return address if a citizen would like his/her passport to be sent by mail.
Please note that passports are issued within 45 days after the application and necessary documents are submitted. Passports issued for minors are valid for 3 years
