An application form (only necessary when requesting the travel document by mail) must be filled out in Georgian language. It is important to note that all questions on the application must be fully completed and signed by the applicant
Submit E-Application Instructions
The following documents must be presented in order to receive a biometric passport:
1) Copy of the applicant’s passport (if applicable);
2) A money order in the amount of $35 U.S. dollars written out to the Embassy of Georgia in Washington DC;
3) 1 passport size photo (3X4 cm).
The documents mentioned above can be submitted in person at the Consulate General or sent by mail. Documents sent by mail, must be accompanied by a prepaid envelope with a return address.
